Coaches: Please submit a separate form for each game or match played.

SPORTSMANSHIP EVALUATION (Copy and Paste Form below)
Coaches: Please evaluate your opponent’s sportsmanship within 48 hours after each league game.  
You will be given a composite score and analysis of all the coaches voting at the next league meeting.  
This report will help you gain a feel for your team’s strengths and weaknesses. 

Teams who forfeit a game or do not vote after each game will not have their choices counted for the sportsmanship trophy
and will be ineligible for awards.   Remember, this must be done within 48 hours after each game.
Rate each team from 5 to 1 for each category. 
5 = Excellent        4 = Good        3 = Average        2 = Needs Improvement        1 = Unsatisfactory
